Smart Preparation: Before each appointment, Hearth automatically compiles recent health data, medication changes, and care notes into a shareable summary. No more scrambling to remember what happened at the last visit.
Real-time Updates: When a family member takes your loved one to an appointment, the rest of the circle gets live updates. "Arrived at Dr. Thompson's office." "Appointment finished — everything looks great." Simple, reassuring, and automatic.
Care Transition Support
The most stressful moments in a care journey often happen during transitions — hospital to home, rehab to outpatient, one provider to another. Important information falls through the cracks, medications get changed without everyone knowing, and care plans diverge.
Hearth's new transition feature creates a structured handoff checklist that travels with your loved one. Every provider, family member, and caregiver in the circle can see what's happening, what needs to happen next, and who's responsible.

We've also added basic nutrition tracking based on your requests. Log meals, track hydration, and note dietary restrictions that caregivers and family members can reference. It's especially useful during recovery periods when proper nutrition is critical.
